I hereby consent to my child participating in the EastBay FC Youth Soccer Tryout event, hosted at Mt. Diablo High School. I understand the nature of the activities involved and acknowledge the risks associated with participation in soccer tryouts.

In consideration for allowing my child to participate in the tryout, I hereby waive, release, and discharge EastBay FC, its directors, officers, employees, agents, and volunteers from any and all liability, claims, demands, actions, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by my child during the tryout event or as a result of participating in the tryout.

I understand and acknowledge that soccer is a physical sport that involves inherent risks, including but not limited to the risk of physical injury. I certify that my child is physically fit and capable of participating in the tryout activities, and I assume all risks associated with their participation.

I further agree to indemnify and hold harmless EastBay FC, its directors, officers, employees, agents, and volunteers from any and all claims, liabilities, damages, or expenses (including attorney's fees) that may arise from my child's participation in the tryout event.

I hereby grant permission for EastBay FC to obtain emergency medical treatment for my child in the event of injury or illness during the tryout event, if deemed necessary. I understand that every effort will be made to contact me or another contact I have listed on the registration form.

By checking the box on the registration form, I have read and understand this waiver and release of liability, and I voluntarily agree to its terms and conditions on behalf of myself and my child.
